Required Volunteer Forms

The Diocese of Allentown and St. Anne Catholic Church require that all volunteers and employees who come in contact with children complete background check forms which can be obtained through the school website:

or the Allentown Diocesan website:

These forms must be submitted to Suzanne Connell at the rectory sconnell@stannebethlehem.org prior to volunteering or employment.
